Primary care
Lincolnshire Integrated Care Board Medicines Optimisation and Prescribing
The ICB Medicines Optimisation Team encompasses a broad skill mix of Pharmacists and Pharmacy Technicians who advise and support quality, safe and cost-effective prescribing, particularly in primary care. Staff offer expert advice on medicines matters relating to contracting and commissioning for the ICB, and ensure that patients are at the centre of their care by encouraging shared decision-making about the medicines they use. An effective Medicines Optimisation Team brings together ICS elements through their role in pathway design for effective medicines delivery throughout the whole patient journey.
A Primary Care Network (PCN) Pharmacy team collaborates with the multi-disciplinary team within General Practice and the extended PCN team. Staff contribute to improving prescribing practice, patient care, and delivery of the Directed Enhanced Services contract prescribing agenda. This includes encouraging the development of better understanding of the principles of medicines optimisation and promoting good practice in line with therapeutic developments, reducing medicines waste and minimising clinical risk associated with medicines taking.
Community Pharmacy In Lincolnshire
There are many community pharmacy organisations in Lincolnshire, including independent business owners with one pharmacy, small groups of pharmacies or those which are part of larger regional or national chains.
Community pharmacists are considered to be the health professional that is most accessible to the public, as they are available to provide person-centred advice about health and medicines when needed, without the need for an appointment. As well as being accessible experts on medicines, key services such as clinical assessment of prescriptions, dispensing medicines and healthy promotion form an important part of the role of the community pharmacy team. Most community pharmacies also provide a range of advanced services too, which allow pharmacy teams to provide more accessible care, using the skill mix of trained pharmacy teams alongside the pharmacist. These include things such as the New Medicine Service, which supports patients to get the most from prescribed medicines, and Pharmacy First, which offers support to patients with minor health conditions and who may need urgent supplies of medicines in emergencies.
Community pharmacy is currently undergoing significant transformation with growing patient-facing service development, supporting the delivery of joined-up, responsive and person-centred community-based health and care services.
